Illinois Alzheimer’s Disease State Survey: Please Complete
The Cognitive Neurology and Alzheimer’s Disease Center (CNADC) has partnered with the Alzheimer’s Association to survey Illinois residents about state programs and services that support those affected by Alzheimer’s Disease and related dementias.
By participating in the annual Alzheimer’s Association Illinois Survey you will provide greater understanding about the challenges faced by those living with dementia, their caregivers, and loved ones as well as help shape state policies addressing the critical needs of those impacted. This online survey, which takes only 10 minutes to complete, will close Friday, May 16.
Through the CNADC, the Alzheimer’s Association, and other partners, the answers you and hundreds of others share in this second annual survey will be presented at conferences, shared with the media, included in government testimony, presented by volunteer advocates to their elected officials, and used to highlight the challenges in dealing with Alzheimer’s disease in new ways. Last year, the survey analysis was used to inform recommendations in Illinois’ new Alzheimer’s Disease State Plan, presented in testimony to the Illinois House Human Services Committee, distributed to the public health community throughout Illinois by the Illinois Public Health Association, and shared with numerous agencies and organizations serving the aging population in our state.
