Care Navigation
Care Navigation provides a series of counseling appointments with a licensed social worker who is specifically trained in issues related to memory loss. Your appointment with a Care Navigator can be a source of guidance, support, education and empowerment as you navigate the challenges and changes associated with Alzheimer’s disease and other forms of dementia. This service is available in some office locations and is now available by phone. This is a fee-based service. Fees are based on ability to pay and will be discussed with you when you schedule your appointment.
Audio Conference
These monthly, one-hour calls are conveniently scheduled so you can attend from home or office. A variety of topics are available. These include coping techniques, disease education and caregiver stress. Pre-registration is required. Once registered, you will receive a toll-free telephone number with instructions through the mail. On the day of the conference, you will call in and join many others who are seeking the latest information on memory loss.
Connections – an Early Stage Support Group
Have you been diagnosed with Mild Cognitive Impairment or an early stage of Alzheimer’s
disease or other dementia? If so, join us by phone for an educational and supportive “virtual” gathering of individuals from across the state with a similar diagnosis. These monthly phone calls will provide an opportunity to connect with others, explore challenges, share insights and learn about community resources. Two groups will be held each month — one for individuals with the diagnosis and one for care partners. Pre-registration and screening is required.
Caregiver Support Group
If someone you love has Alzheimer’s disease or another form of dementia, consider joining a support group to meet with others, share experiences and gain support from each other. If a support group does not meet near you or meeting times conflict with your schedule, you can still get connected with others through our caregiver support group via phone.
