At the Alzheimer’s Association® Greater Illinois Chapter, we understand that not everyone is able to attend a program or support group or receive a service in person. Care Navigation

Care Navigation provides a series of counseling appointments with a licensed social worker who is specifically trained in issues related to memory loss. Your appointment with a Care Navigator can be a source of guidance, support, education and empowerment as you navigate the challenges and changes associated with Alzheimer’s disease and other forms of dementia. This service is available in some office locations and is now available by phone. This is a fee-based service. Fees are based on ability to pay and will be discussed with you when you schedule your appointment.
Audio Conference

These monthly, one-hour calls are conveniently scheduled so you can attend from home or office. A variety of topics are available. These include coping techniques, disease education and caregiver stress. Pre-registration is required. Once registered, you will receive a toll-free telephone number with instructions through the mail. On the day of the conference, you will call in and join many others who are seeking the latest information on memory loss.
Connections – an Early Stage Support Group
Have you been diagnosed with Mild Cognitive Impairment or an early stage of Alzheimer’s
disease or other dementia? If so, join us by phone for an educational and supportive “virtual” gathering of individuals from across the state with a similar diagnosis. These monthly phone calls will provide an opportunity to connect with others, explore challenges, share insights and learn about community resources. Two groups will be held each month — one for individuals with the diagnosis and one for care partners. Pre-registration and screening is required.
Caregiver Support Group
If someone you love has Alzheimer’s disease or another form of dementia, consider joining a support group to meet with others, share experiences and gain support from each other. If a support group does not meet near you or meeting times conflict with your schedule, you can still get connected with others through our caregiver support group via phone.

The Alzheimer’s Association highly trained and knowledgeable Helpline staff and volunteers
provide reliable information and support to all those who need assistance, with translation
services available in 170 different languages, 24 hours a day, 7 days per week. We serve people with Alzheimer’s disease or other forms of dementia, caregivers, health care professionals and
the public. Contact us to talk with a caring professional about your questions and concerns as
well as to understand more about memory loss, dementia and Alzheimer’s disease and discover resources available in your community.

Join us Wednesday, March 13 for the 2013 Illinois Action Summit!
During this important event, we will encourage our Illinois legislators to understand the importance of prioritizing Alzheimer’s disease among our State’s policies. Your presence and voice provide the greatest opportunity to ensure that the needs of Alzheimer’s patients, family members and caregivers are addressed. Please consider taking the time to be a part of this critical movement!
At the beginning of the Summit, we will provide information on the Association’s policy priorities for 2013 as well as the messages we will ask advocates to carry to legislators throughout the afternoon. Strategies and tools for a successful visit will also be shared.
Following lunch, we will host a meet n’ greet with Lt. Governor Sheila Simon in her Springfield office. This event will provide an opportunity for you to engage your legislator in a fun and informal setting. We will also visit many other members of both the Illinois Senate and House of Representatives, and you will have the chance to share your personal stories and ask for support for Alzheimer’s initiatives.
Transportation to the Capitol will be arranged from a variety of locations. A small donation is requested to help cover lunch, snacks, transportation and all advocacy materials.
